Patient management is a category with a boundary problem. To some vendors it means the whole practice system, scheduling and billing and record together. To others it means a layer that sits on top of the system you already have and does booking, reminders, forms and payments better than it does. Those are different purchases with different prices and different risks, and comparing across them is the commonest way this shortlist goes wrong.

Layer or system: decide first
If your clinical record is fine and your problem is no-shows, forms on clipboards and a phone that never stops, you are buying a layer, and the question is whether it integrates with what you already run. If the record itself is the problem, you are replacing the system and the layer question is premature. Write down which one you are on the first line of the sheet, because it decides which vendors belong on it at all.
If it is a layer, integration is the whole score
Ask which version of your existing system it connects to, whether the connection is read-only or writes back, what it does not sync, and how long the connection has been live for practices on your version. A layer that cannot write an appointment back is a diary somebody has to copy, and that is worse than the problem you started with. Get the answer per product per version, in writing, and score it.
The three things worth measuring
No-show rate before and after, which the vendor should be willing to discuss with a reference practice; how much front desk time is returned per week; and how much is collected at the point of service rather than chased. Ask for those in a reference call rather than from a case study, and put your own current figures on the sheet first so you have something to compare the answers against.
