The figures come from the numbers you type into the worksheet and the arithmetic printed next to each answer, and the worked example is there so the page is not blank before you start. Where a page cites a rule or a certification programme, the link goes to the document itself rather than to a summary of it.

An EHR comparison is only worth running once you have decided what you are comparing on, and that order gets reversed constantly: people collect four vendor feature lists first and then try to find a question those lists happen to answer. Start at the other end. Write down the handful of things you will walk away over, decide what each is worth, and only then let a vendor talk. Everything below is about getting to that list, and the comparison sheet on this site does the counting once you have it.

Write the walk-away list before anybody demos

Six to fifteen requirements you will not trade: the specialty templates you already work in, the lab or imaging interface you pay for today, e-prescribing with controlled substances if you write them, and whatever your billing company needs out the other end. If you cannot say why a line is on the list, it is a nice-to-have.

Weight them, then hold the weighting still

Three points for a must-have, one for a nice-to-have is enough arithmetic for a decision this size. Fix the weights before the first demo and do not adjust them afterwards to make a favourite win, because that is the failure this whole exercise exists to prevent.

Score every vendor on the same rows, with the same people

The lead clinician, the practice manager and whoever runs billing each score the same sheet. Where two of them disagree by more than a point on a must-have, that row is a question for the next demo rather than an average to take.

Check certification separately from the sales conversation

Whether a product is certified, and to which criteria, is a matter of public record on the Certified Health IT Product List rather than of a claim in a deck. Look the product up by name and by version, because a vendor can be certified on one edition and not the one you are being sold.

What an owner asks before filling in the EHR comparison for a small practice

How many vendors should be on the sheet? Three to five. Below three you have no spread to judge against; above five the scoring stops being done carefully and the demos alone eat a fortnight. Cut the longlist on the must-haves before anybody books a call.

Do you tell me which EHR is best? No, and we never will. Siftvo publishes no ranking and no score of its own, and no vendor pays to appear here. The only score on this site is the one your own weighting produces from your own inputs.

What if a vendor will not answer a requirement in the demo? Score it zero and say so on the sheet. A requirement a vendor will not demonstrate is a requirement they do not meet yet, and the version of that answer you get after signing is worse.
