The number that decides whether you can afford a system is the whole term: the subscription for every seat that actually needs one, plus the implementation and data migration fee, plus the training days, over the years you will realistically stay. Two quotes with the same headline rate can be tens of thousands apart once those are in, and the difference is usually in who counts as a seat.

Count every seat, then ask what each one is charged at
Clinicians, front desk, billing, the practice manager and the part-time locum all need logins. Some quotes charge support staff at a fraction of the clinician rate and some charge them the same, and that single answer moves the total more than the headline rate does.
Get the one-off fees in writing before you compare anything
Implementation, data migration from the old system, interface build for the lab or the imaging centre, and the training days. These are quoted separately, land in the first quarter, and are the part of the number that surprises people.
Compare on the same term and the same seat count
Put every vendor on the same number of years and the same seats before you look at a total. A five-year total against a three-year total is not a comparison, and vendors quote in whichever shape flatters them.
Divide the whole term back per clinician
The per-clinician figure over the term is the one a partner or a board understands without translation, and it is the number that makes a large implementation fee visible next to a low monthly rate.
